Re-SET: Ecosystem Enabling Approach to Addressing Cancer Disparities in Philadelphia

Re-SET (Results – Screening, Early Diagnosis, and Treatment) is a learning collaborative designed to improve cancer care in Philadelphia by addressing health equity and system barriers to access. The initiative supports evidence-based interventions, including patient education and reminders, navigation, transportation, and translation services. It also emphasizes data-driven approaches, virtual learning communities, sustainable process improvements, and centralized navigation resources tailored to participating health centers.

Through a competitive RFP process, six organizations were selected to receive funding and expand their cancer screening efforts. Each sub-grantee works to achieve two or more of the following goals:

  • Increase cancer screening
  • Support follow-up outreach to help patients access appropriate care
  • Facilitate effective communication between primary care and larger health systems to improve awareness and access to screening and care
  • Support non-medical needs of individuals seeking cancer diagnosis or treatment

This work led to the creation of the Cancer Care Navigator Collaborative, a peer-to-peer working group for care coordination staff and navigators who support patients across the cancer care continuum, from screening and prevention to treatment and survivorship. 

The purpose of the collaborative is to provide a space for staff to share best practices, experiences, tools, and resources, while addressing both system-level and individual barriers to strengthen patient-centered care across the cancer continuum.

Events that are offered focus on networking and relationship-building, open discussion platforms on issues that impact cancer care navigation, and skill-building trainings.

Membership & Participation:

  • Open to navigators and care coordination staff from all organizations involved in cancer care, from screening and prevention to treatment and survivorship

About: This event will explore common causes for patient resistance to scheduling and participating in cancer screenings. By exploring the reasons and perspectives patients share, we will identify opportunities to use supportive language, validation strategies, and relationship-building approaches when engaging clients who may feel resistant to screening.
